CPD - ASSESSMENT & EVIDENCE REFERENCING
Summary Lists
The Learner
For each Unit there must be:
- A Unit Summary Form
- An Evidence Reference Form
- Assessment Report(s) from the Assessment(s)
- Assessment Summary (or Summaries) from the Assessment(s)
Where an assessment is used to assess more than Unit these documents are simply referenced by a number or letter code - do not copy each document for every Unit or Element you need to use them!
The Assessor
For each Assessment there must be:
- An Assessment Plan
- An Assessment Report
- An Assessment Summary
The Portfolio
Each Learner must create a portfolio to contain:
- A CV
- A Job Description
- Any Needs Analysis, Development plans or Administrative items associated with the CPD Programme
- An Index of Units Assessed
- Each Unit assessed (ie a Unit summary sheet, Evidence Reference sheets, Assessment forms)
- Any evidence you wish to keep with these assessments.
